RTT: improve debugger responsiveness and polling error reporting - #2054

Report the first RTT polling failure and recovery in gdbserver and run. Propagate channel transfer errors and suppress repeated warnings.
Prevent RTT discovery from occupying the service loop and delaying GDB commands. Cache the ELF symbol address and give the client thread time to process commands between discovery attempts.
