// A CSV that fails because of its OperatorGroup loses what OLM created for it:
// the Deployments and webhook configurations of the installer, and the RBAC
// steps of its InstallPlan. A step whose object the cleanup found, verified
// as owned by the CSV and deleted is marked NotPresent in the InstallPlan
// status while the plan stays Complete. When the CSV is Pending again, a
// plan with NotPresent steps goes back to Installing and the catalog-operator
// creates those objects again. The InstallPlan status is the only record of
// the cleanup, and only the controllers write it.
//
// InstallPlan execution is at least once: a catalog-operator sync that works
// from a stale Installing copy of a plan that is already Complete creates the
// objects again. The cleanup therefore deletes on every sync of the failed
// CSV, marks or not, and never skips a step because it is already marked.

// recovery creates again. ServiceAccounts are kept: the OperatorGroup scoped

Refresh plan.ResourceVersion after the first UpdateStatus. The mark rollback at Line 203 otherwise always conflicts.

Line 178 discards the object that UpdateStatus returns. The API server increments the InstallPlan resourceVersion on that write. The second UpdateStatus at Line 203 then sends the stale resourceVersion from the original List. When changed and unconfirmed are both true, a real API server rejects that write with 409 Conflict. The fake clientset does not check resourceVersion, so TestFailedCSVCleanupObjectGoneBeforeDelete passes but does not exercise this case.

Result: the rollback of the mark (v.step.Status = StepStatusPresent) is never stored. The next sync cannot repair it. getStepObject returns NotFound, the loop skips the step, and the step stays NotPresent. reinstallPlanSteps then reopens the plan, and the catalog-operator recreates an object that another actor deleted. The comment at Lines 193-194 says the code must prevent exactly this.

The trigger needs a delete race. The failure is deterministic once that race occurs.

Review comment at @pkg/controller/operators/olm/csv_cleanup.go around lines 177
- 182:
Update the first UpdateStatus call in the changed block to retain its returned
InstallPlan and copy the returned ResourceVersion onto plan before the later
rollback UpdateStatus call. Preserve the existing error aggregation and early
return behavior.
