Review the fan-out of workflow_run to all open PRs.
Every pull_request_review event triggers an evaluation of every open PR. This includes human reviews and routine CodeRabbit COMMENTED reviews. Each evaluation sets pending statuses and makes several paginated API calls. The result is status flapping on unrelated PRs and rate-limit pressure on GITHUB_TOKEN as the open-PR count grows.
Line 42 also fails the targets job when more than 256 PRs are open. Evaluation then never runs. A success status from an earlier approval stays in place after a dismissal. This fails open, which contradicts the fail-closed intent.
Consider these options:
Narrow the candidate PRs with trusted metadata, for example PRs whose head SHA or branch matches the workflow_run payload.
Add an alert or a documented manual workflow_dispatch runbook for the overflow case.

Review comment at @.github/workflows/risk-gate.yml around lines 35 - 44:
Update the `workflow_run` branch to evaluate only PRs matched using trusted head
SHA or branch metadata from the event, rather than fanning out to every open PR.
Replace the 256-PR hard failure with an explicit alert or documented
`workflow_dispatch` recovery path so overflow cannot silently leave stale
approval statuses in place.

Restore the actor check for owner-approved labels.
The gate now accepts an owner-approved label without checking who applied it. Any user or agent with permission to label the PR can open the high-risk gate. The previous workflow rejected labels not applied by the owner. The repository policy requires that restriction, and the gate should enforce it in code. Restore the previous label-history check:

Update the `owner-approved` label check so approval is accepted only when label

reject labels applied by other actors or subsequently removed.

Restrict the coderabbit-queue marker to its trusted producer.
The check accepts a matching marker from any commenter. A PR author can post it and suppress the automated CodeRabbit request for the current head. This does not bypass the gate, but it removes the CodeRabbit approval path.
The workflow’s own comment is a separate coderabbit-review-head marker authored by github-actions[bot], so filtering the queue marker does not affect it. The repository does not identify the producer of coderabbit-queue; use that producer’s trusted identity rather than assuming it is github-actions[bot]. An OWNER/MEMBER check alone would still allow a member who authored the PR to post the marker.

Update the `comments.some` check for the `coderabbit-queue` marker to accept it
only when authored by the trusted identity of the queue marker’s actual
producer; do not assume it is `github-actions[bot]` or rely only on OWNER/MEMBER
permission, and ensure the PR author cannot suppress the request by posting the
marker.

## Do not
- Only the owner, or a reviewer explicitly authorized by the owner, may apply the `owner-approved` label; coding agents must never apply it to their own PRs.
- Do not add a live QMT or miniQMT broker connection; this repository has no
configured broker target.
- Do not report a `blocked` adapter result as submitted or filled.
