test: add and complete Jasmine/Karma unit tests across TM-UI - #167

- karma.conf.js with headless Chrome and html/lcov/json-summary coverage; coverage excludes Common-UI, mocks, environments and test helpers - src/testing/test-utils.ts: autoSpy, language/session/dialog/confirmation mocks; unhandled RxJS errors are reported against the running spec instead of aborting the run - test:ci npm script - replace Angular CLI "should create" stubs and broken legacy specs with behavioural specs across core, registrar, nurse-doctor, lab, pharmacist, data-sync and login flows - new specs for TM-only cancer screening components (examination, history, vitals, case record, case sheet, refer) and NCD screening 3388 specs passing; line coverage 84.4%.
