Validate tbuId against the village on both write paths.
The save and update endpoints accept a caller-supplied tbuId without checking the village’s facility-to-TBU relationship. Save then upserts by (villageId, tbuId, designatedPhiId). Update can change either the village or TBU and applies the same unchecked tuple.
The repository derives a village’s TBU from village → facility → TBU for unmapped results, but filters mapped results by the persisted mapping TBU. A noncanonical TBU can therefore hide the active mapping from the village’s canonical TBU or district result while still excluding that village from unmapped results. A later save with the canonical TBU can also create another active row instead of updating the existing PHI mapping.
Add this validation in saveDesignatedPhi and in updateDesignatedPhi whenever villageId or tbuId changes. This validation is required in this service independently of device enrollment.

Block deviceName changes while the device has active village mappings.
updateDevice changes deviceName without checking existing active mappings. The "one active NAAT and one active XRAY device per village" rule is enforced only in enroll, through findActiveByVillageIdsAndDeviceName.
Example: device A (NAAT) serves village V, and device B (XRAY) also serves V. An update that changes A to XRAY leaves V with two active XRAY devices. The Ni-kshay job and findPhiByLocation then return two logins of the same type for V. The job can pick the wrong PHI login for that village's beneficiaries.
Reject the change while the device has active villages. Alternatively, re-run the conflict check for the new type.

Derive the village location during enrollment.
enrollVillages checks only that villageId exists, then stores the submitted districtId, tbuId, and facilityId. getPhiByLocation filters these stored values directly. A valid authorized request can therefore place a village under another location, return its device login for that location, and omit it from the village’s canonical location. Load the village’s canonical facility/TBU relationship during enrollment and reject or replace mismatched location values before saving.
